caffeine pills are a stamina item, when you get to level 5 you'll be able to buy a house. Once you do, a new meter will appear on the right that says stamina.
Once you have a house, you'll be able to train your stats (speed/defense/strength) and each time you train. Your stamina goes down, the pill will help get that back up so you can keep training.

Yeah you gotta watch that... If your HP starts running low and you're low on energy you can kill yourself be attacking a level 1, so make sure and watch them both.
